New Patients: What to Expect
If you haven’t printed our intake forms from this website and filled them out at home, you will have forms to fill out when you arrive. We will make a copy of your insurance card. Bring a list of medications and past relevant imaging (x-ray, MRI, CT)
Please wear loose fitting clothing.

 If you have questions concerning whether your insurance covers Chiropractic Care, please call the phone number on the back of your insurance card and inquire about Chiropractic coverage.
We take Workers Compensation and No-Fault cases as well
We DO NOT accept any Medicaid/Option products.
We also have a self-pay option for those who do not have Chiropractic coverage. Please call the office for these fees.
